What this website collects
When you submit the prescription transfer request form, we collect only your name, phone number, the best time to call, and, if you choose to provide it, your email address. We use this information solely to call you back about your request. Please do not enter medication names, prescription numbers, or other health information into the form. A pharmacist collects those details by phone.
Form submissions are processed by our website host, Netlify, and delivered to pharmacy staff. Submissions are deleted from the website system once they have been handled.
Your health information
Information you share with our pharmacists in person or by phone is protected health information under HIPAA and is governed by our Notice of Privacy Practices, not by this website policy. This website itself does not store health records.
Analytics
We may use Google Analytics to understand how visitors use our site (pages visited, time on site, general location at the city level). This data is aggregated and does not personally identify you. We do not send form contents to analytics, and we do not use analytics on our form confirmation page. You can opt out with the Google Analytics opt-out browser add-on.
Cookies and local storage
